The Phase 2 Teacher Assessment Sheet is a comprehensive tool used to evaluate a child's understanding and application of phonics at Phase 2 level. The assessment is divided into several sections, each designed to test different phonetic skills. The first section, Grapheme Recognition, requires the child to identify phonemes associated with given graphemes. The teacher will record a 'yes' or 'no' depending on the child's response, with the activity being halted after 60 seconds or if the child experiences significant difficulty.

Following this, the Sounding Out and Blending section tests the child's ability to articulate individual phonemes within words and then blend them to form the word. For example, the phonemes /t/ /a/ /p/ are blended to say "tap." The Segmenting section asks the child to break down spoken words into their constituent phonemes. Tricky Words assessment challenges the child to read common but irregular words that cannot be sounded out using standard phonetic rules. Nonsense Words evaluation involves the child attempting to read made-up words to demonstrate their decoding skills. Lastly, the Eye Tracking portion assesses whether the child reads different text sizes with equal proficiency, which can indicate the need for specific eye tracking exercises. Each section is time-bound, with a 60-second limit, and the teacher offers assistance where necessary, marking a 'no' for any help given.
