Intervention Tracking Sheet - Resource

The Intervention Tracking Sheet is a resource designed to monitor the progress of children who require additional support with phonics. It is a structured document that records a child's journey through the Synthetic Systematic Phonics (SSP) programme. The sheet begins with a section to fill in the child's name and year group, followed by a record of the phases they have completed within the SSP programme. A baseline assessment date is also noted to establish the starting point of the intervention.

The baseline assessment is critical in identifying specific areas of difficulty a child may have. It includes various tests to pinpoint challenges such as auditory processing weakness (APW), optilexia, short term memory weakness, eye tracking weakness, and contrast sensitivity. Each area has a score threshold that determines the recommended starting stage within the programme. For instance, if a child scores 4 or above in APW, they should begin at Stage 1. The sheet provides clear guidance on additional exercises or adjustments, such as eye tracking exercises or the use of a coloured overlay for reading if required. The subsequent section of the tracking sheet is dedicated to documenting the child’s progress through the stages of the programme. After completing each stage, the child takes a post-assessment to decide if they are ready to advance. The dates and outcomes of these assessments are recorded, providing a clear history of the child's development and ensuring they receive the tailored support they need to improve their phonics skills.
